The original evidence pointed more towards plants and olive oil than fish and chicken when it comes to living longer.

The Mediterranean diet has become one of the most widely cited dietary patterns in nutritional research, frequently presented as a model that combines an abundance of vegetables, legumes, whole grains, and olive oil with regular servings of fish and, in more recent iterations, lean poultry such as chicken. However, in one of the earliest large-scale prospective studies to quantify adherence to the Mediterranean diet and relate it to our chances of survival, poultry was grouped with red meat as a detrimental component, meaning lower intake of both contributed to a higher diet score (Trichopoulou et al., 2003). Framing poultry and fish as beneficial has shaped public health messaging, clinical advice, and food industry marketing for decades. But when we look more closely at the evidence, the picture is more complicated. Fish and chicken may be better choices than red or processed meat, but that does not mean they are the reason the Mediterranean diet became linked with living longer.

Drawing on 22,043 adults recruited across Greece as part of the European Prospective Investigation into Cancer and Nutrition (EPIC), the study followed participants for a median of 44 months and used a validated nine-component Mediterranean diet score to assess dietary adherence. A two-point increment in that score was associated with a 25 percent reduction in total mortality, with significant inverse associations for both coronary heart disease mortality and cancer mortality. These are substantial effects for a dietary pattern in a general population sample.

What is less frequently discussed is what the individual components of the diet revealed. When each dietary variable was examined separately, the authors found that only two measures were independently predictive of total mortality: intake of fruits and nuts, and the ratio of monounsaturated to saturated lipids. Amongst the other dietary sources only vegetables, legumes, cereals, and olive oil, showed a trend toward lower mortality risk although none reached statistical significance individually. The authors acknowledge this directly, noting that individual components may carry small effects that only become detectable when integrated into a composite dietary score, and that the composite can capture cumulative exposure in ways that single-food analyses cannot.

Fish is worth examining here specifically, because it sits at the heart of how the Mediterranean diet is usually communicated. Fish was included in the nine-component score as a beneficial element, and its addition to the scoring framework was justified partly by evidence at the time linking fish consumption to reduced coronary heart disease risk. But when fish intake was analysed individually in this study, it showed no meaningful association with survival. In this cohort, fish did not independently drive the mortality benefit observed.

The same research group returned to this question in a 2009 study using the same Greek EPIC cohort, now with 8.5 years of follow-up and over a thousand deaths, designed specifically to quantify each component's contribution to the survival benefit. Fish and seafood was the only component associated with a non-significant increase rather than a decrease in the mortality ratio. The dominant contributors to living longer were low meat intake, high vegetable consumption, fruits and nuts, olive oil, and legumes (Trichopoulou et al., 2009).

Poultry is even more revealing. In their scoring system, poultry was not treated as a neutral or protective food. It was grouped with red meat under the "detrimental" category, meaning that higher poultry consumption was associated with a lower Mediterranean diet score. This is entirely at odds with the modern presentation of the Mediterranean diet as one that incorporates lean chicken as a beneficial animal protein alongside fish.

The interpretation that follows is not that fish and poultry are harmful. It is that the evidence from this foundational cohort study does not position them as the protective mechanisms of the Mediterranean dietary pattern. The more plausible drivers of the observed survival advantage are the overall dietary structure: a pattern dominated by plant-based foods, characterised by high intakes of legumes, fruits and nuts, and vegetables, low intakes of red meat and full-fat dairy products, and a fat profile shaped by olive oil rather than saturated animal fats. The ratio of monounsaturated to saturated lipids, one of the two individually significant predictors in the study, is essentially a proxy for the degree to which olive oil has replaced butter, processed meats, and high-fat dairy in the diet.

Fish and chicken became part of the Mediterranean diet story largely through a different route: comparison. In nutritional epidemiology, populations eating more fish and poultry and less red and processed meat tend to show better health outcomes, and this is a consistent and well-replicated finding. But better than red meat is not the same as independently protective. A 2018 meta-analysis of thirty prospective cohort studies covering over 225,000 deaths found the same pattern at scale: fish showed a null association with all-cause mortality across the evidence base, while fruit, vegetables, and low meat intake carried the clearest signals (Eleftheriou et al., 2018). The comparison conceals what is actually doing the work, which in the original Greek cohort data points strongly to a plant-rich, olive-oil-dominant dietary pattern in which low meat intake of any kind is part of the broader structure, not a feature that can be disaggregated from it.

This matters because the Mediterranean diet has not only been studied. It has been translated, packaged, taught, and sold. In that process, the message has often shifted from "eating a plant-rich diet and keeping meat intake low" to "add fish and lean chicken to a healthy plate." That may be easier to sell, and easier for many people to adopt, but it is not quite the same message. The original Greek cohort data do not suggest that fish or chicken were the reasons for any survival benefit observed. The clearer signal was a plant-dominant pattern, rich in vegetables, legumes, cereals, fruits and nuts with olive oil displacing saturated fats from meat and dairy. In plain terms, the Mediterranean diet was not made healthy by adding chicken and fish. It was made healthier by what it pushed off the plate.
